Injury Disclosure & Medical Acknowledgement

I, the undersigned parent/guardian, confirm that I have fully disclosed any and all known medical conditions, injuries, or physical limitations that may affect my child’s participation in hockey training activities.

I understand that participation in hockey involves physical exertion and inherent risk, and I acknowledge that it is my responsibility to inform the program of any changes to my child’s health status prior to participation.

I confirm that my child:

* Is physically able to participate in hockey-related activities, unless otherwise noted above
* Has no undisclosed injuries or medical conditions that would prevent safe participation

I understand that failure to disclose relevant medical information may increase the risk of injury.

I acknowledge that the coaching staff will take reasonable precautions but cannot be held responsible for injuries resulting from undisclosed conditions.

Assumption of Risk & Liability Waiver

I, the undersigned parent/guardian, understand and acknowledge that participation in hockey training and related activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment, or surfaces, and other potential hazards that may result in injury.

On behalf of my child, I voluntarily assume all risks associated with participation in this program.

I hereby release, waive, and discharge Rise Hockey, Coach Brian McAllister, and any associated staff, instructors, or facility operators, from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in the program.

I understand that this waiver applies to any injury resulting from participation, including those caused by negligence, to the fullest extent permitted by law.

I confirm that my child will follow all instructions and safety guidelines provided by the coaching staff.

I acknowledge that I have read and fully understand this waiver and agree to its terms voluntarily.
